Rotavirus RV5 - 3 doses
Diphtheria, tetanus, acellular pertussis (DTaP) - 5 doses by 6 years old
Haemophilus influenzae type b (Hib) - 3 or 4 doses
Pneumococcal conjugate (PCV13) - 4 doses
Inactivated oliovirus (IPV <18yrs) - 4 doses
Influenza - 1-2 doses per year until 8 years old, then 1 per year
